Pico laser for nevus of Ota: realistic expectations

Nevus of Ota (and the related nevus of Ito) is a benign pigmented birthmark that appears as bluish-gray or brown discoloration around the eye and on the face. It’s notably difficult to treat with most lasers, but pico lasers — particularly with the 1064nm wavelength — are currently the best non-surgical option.

What is nevus of Ota?

Nevus of Ota is a congenital pigmented birthmark caused by dermal melanocytes — pigment cells that should have migrated to the skin’s surface but got stuck in the dermal layer. Characteristics:

Older Q-switched lasers had been used for nevus of Ota since the 1990s. Pico lasers offer:

Eye involvement

Nevus of Ota involving the sclera (white of the eye) is more complex. Treatment near or on the eye requires corneal protective shields and specialized expertise. Some clinics won’t treat ocular nevus of Ota — find a specialist if needed.

Children

Treatment can begin in childhood, though many providers wait until older childhood or adolescence. Discuss timing with a pediatric dermatologist.

Pigmentation history

Patients with nevus of Ota are at higher risk of post-inflammatory hyperpigmentation. Patch testing strongly recommended before any full treatment session.

Combined with other treatments

For very deep or extensive nevus, some practices combine pico laser with Q-switched laser approaches alternated across sessions.
