Here’s a candid look at what clinics don’t tell you about pricing.

The headline price isn’t the total price

“Tattoo removal starting at $99” is a real headline on many clinic websites. The “starting at” does a lot of work in that sentence.

Common ways the headline price hides the real cost:

  • Applies only to tattoos under 1 square inch (almost nothing qualifies)
  • Doesn’t include the mandatory consultation fee ($75-$200)
  • Doesn’t include numbing cream ($25-$75 extra per session)
  • Doesn’t include patch test ($100-$200)
  • Increases significantly for the additional area
  • Requires booking a 6-session package upfront to get
  • Drops to a “limited time only” promotion that expires

Always ask for the total estimate for your specific case — not the starting price.

Per-session vs per-square-inch pricing

Most clinics use one of these models:

Per-session flat pricing

You pay a flat per-session fee based on overall tattoo size (small/medium/large). Pros: simple, predictable. Cons: small-tattoo patients sometimes overpay; large-tattoo patients sometimes underpay.

Per-square-inch pricing

Common at dedicated tattoo removal practices. Pays a base fee plus per-inch. Pros: fair scaling for your specific size. Cons: harder to compare across clinics; can add up faster than expected for larger work.

Hybrid

Per-session with surcharges for additional areas or treatments above a base threshold. Less common but increasing.

Package vs pay-per-session

Most clinics offer package discounts:

  • 3-session: 5-8% off
  • 6-session: 10-15% off
  • 10-session: 15-20% off

The math: 6 sessions × $300 = $1,800. With 12% package discount = $1,584. Savings of $216.

The risk: if the clinic isn’t the right fit, the package is non-refundable at most places. Or refundable with a significant cancellation fee.

Smarter approach for most patients: pay-per-session for the first 1-2 visits, then upgrade to the package if you’re confident in the clinic.

What’s actually included in a session price

This varies enormously. Quality clinics typically include:

  • Numbing cream applied 30+ min before
  • Cold air cooling during treatment
  • Aftercare ointment and bandages
  • Written aftercare instructions
  • Follow-up phone access

Lower-end clinics charge separately for each of these, sometimes silently. Read the consent form carefully or ask for the itemized estimate.

What’s almost never included

  • Initial consultation (some clinics free; others $50-$200)
  • Patch test ($100-$200 typical)
  • Touch-up sessions (some include 1, most charge full price)
  • Post-treatment products beyond basic ointment
  • Any follow-up consultations beyond initial

The “discount” problem

Clinics offering 30%+ discounts on packages have a math problem. Either:

  1. The “regular price” is inflated to make the discount look bigger
  2. The clinic is desperate for business and may be cutting corners
  3. The package is structured to lock you in before you can compare

None of these are great signals. Be skeptical of huge discounts.

Geographic price variance

Same procedure, different cities:

  • NYC, LA, SF: 20-40% premium over national average
  • Major metros (Boston, Chicago, Miami, Seattle): 10-20% premium
  • Regional centers (Denver, Phoenix, Nashville): Near national average
  • Smaller metros and suburbs: 10-20% below national average

If you’re flexible on location, traveling to a less expensive metro for premium care can save thousands over a treatment course. Not common for tattoo removal but worth knowing.

The “we’ll match competitor prices” offer

Some clinics offer to match competitor pricing if you bring a quote. Worth knowing about, but be careful:

  • They may match the per-session number but not the included services
  • They may match this session and bump the next
  • They may match if you commit to a 10-session package

Get all matching terms in writing.

Financing reality

  5. That’s your monthly budget target

Bottom line

Pricing in the pico laser market is more variable and less transparent than most other elective procedures. Get specific written quotes, ask what’s included, be skeptical of headline pricing and big discounts, and budget for the full course rather than just the per-session number.

The cheapest clinic is rarely the best value. The most expensive clinic isn’t always premium care. The clinic with clear, honest pricing — explained upfront with everything itemized — is usually the one to trust.
